Medical Advantage Primary Care Practices Achieve Patient Centered Medical Home Designation

EAST LANSING, Michigan, July 12, 2017 – Primary care practices in Medical Advantage’s physician organization, the Consortium of Independent Physician Associations (CIPA), recently received 129 Blue Cross Blue Shield of Michigan (BCBSM) Patient-Centered Medical Home (PCMH) designations. CIPA’s 129 designations mark a 12 percent increase over 2016 results.

BCBSM’s PCMH program is a comprehensive approach that creates care teams that work with patients to keep them healthy while monitoring their care on an ongoing basis. The PCMH program uses registries to track and manage conditions like asthma and diabetes.

CIPA consists of many sub-physician organizations representing 900+ physicians throughout Michigan. CIPA members include individual practices, group practices, rural health clinics, Federally Qualified Health Centers, and physician hospital organizations. Most CIPA physicians are independent practitioners.

Medical Advantage’s practice transformation team works with CIPA practices to help them achieve PCMH status. The team works onsite with practice staff to implement various capabilities to qualify for PCMH designation. In addition to CIPA, Medical Advantage provides management and advisory services to Professional Medical Corporation (PMC), a Physician’s Organization in the greater Flint, MI area. Medical Advantage’s transformation team helped 67 PMC practices achieve PCMH designation. This brings the total number of Medical Advantage-associated practices with a PCMH designation to 196. Since 2009, Medical Advantage has assisted in over 1,000 practice transformations. Last year, Medical Advantage helped its physician organization clients earn $31 million in additional revenue from fee schedule increases and financial incentives.

Linda Mackensen, Medical Advantage’s Healthcare Transformation Director, leads the team and works directly with care teams to improve quality of care through implementation of patient-centered capabilities. “As we help practices implement more proactive, patient-centered capabilities, patients, providers, and payers all benefit,” Mackensen said. “These improvements increase care quality, decrease unnecessary costs and help practices stay financially viable.
